Transgender Cosmetic Surgery in Las Vegas
Facial feminization, MTF breast augmentation, FTM top surgery, and body contouring — one surgeon, one office, performed by Dr. Robert J. Troell, MD, FACS
Gender-Affirming Cosmetic Surgery, Under One Roof
Transgender cosmetic surgery — also called gender-affirming or gender-confirmation surgery — at Troell Cosmetic Surgery covers the face, chest, and body: facial feminization, MTF breast augmentation, FTM top surgery, tracheal shave, and body contouring built around the waist-to-hip relationship. Every procedure is performed by Dr. Robert J. Troell, MD, FACS in Las Vegas, and every consultation and follow-up happens with him, in the same office.
We are an LGBTQ+ friendly and transgender-safe practice. That is not a page written for search engines — it has been our standing policy, stated on our homepage, before this page existed. Respectful, confidential care, your name and pronouns as you give them, and an honest conversation about what surgery can and cannot do.
What We Offer — Feminizing and Masculinizing
Gender-affirming surgery is not one operation — it is a set of decisions about which features carry the signal you want to change. Dr. Troell’s work covers three areas:
The Face
Facial feminization surgery (FFS): brow and forehead, feminizing rhinoplasty, custom chin and jaw implants, facial fat grafting, tracheal shave. For transmasculine patients, the same skills run in reverse — custom jaw and chin implants that build angular, masculine structure.
The Chest
MTF breast augmentation with implants or stem cell–enriched fat transfer — and FTM top surgery, masculinizing chest surgery planned to your chest size, skin quality, and scar preferences.
The Body
Body feminization: hip contouring with fat grafting or hip implants, Brazilian butt lift, and VASER waist sculpting — the procedures that change the waist-to-hip line.
What We Do Not Do
Dr. Troell does not perform genital surgery (vaginoplasty or phalloplasty), voice surgery, or hormone management. Gender-affirming care is a field where honesty about scope matters more than anywhere else in cosmetic surgery — a practice that stretches past its competence is a practice you cannot trust with your face. If part of your plan is outside what we do, we will tell you at consultation, plainly.
What we do perform — the facial, chest, and body procedures on this page — sits squarely inside the work Dr. Troell has done for more than 30 years: facial plastic surgery, breast surgery with implants and fat, and body contouring, including published, peer-reviewed work on breast fat grafting and aesthetic hip implants.
Why Transgender Patients Choose Dr. Troell
Gender-affirming results are judged on subtlety: a brow that reads soft rather than operated, a chest that matches the frame it sits on, hips that follow a natural line. That is a planning skill before it is a surgical one — and it rewards a surgeon who designs from your anatomy rather than from a template.
Dr. Robert J. Troell, MD, FACS is a Diplomate of the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS), a Diplomate of the American Board of Cosmetic Surgery (ABCS), and a Diplomate of the American Board of Otolaryngology – Head and Neck Surgery, with over 30 years in practice and training at the University of South Florida and Stanford University, where he later served as a Clinical Professor. He has held an active Nevada medical license since 2001 and has practiced in Las Vegas for more than 20 years. You can review his published work and credentials and decide for yourself.
Consultations are with Dr. Troell himself — he is the only surgeon at the practice, and the person who examines you is the person who operates. Consultation, surgery, and every follow-up visit happen at the same office on South Fort Apache Road.
A Self-Pay Practice — Said Plainly
Troell Cosmetic Surgery does not bill insurance — for any procedure, for any patient. We state this first because many people researching gender-affirming surgery are comparing insurance-covered hospital programs, and you deserve to know how this practice differs before you invest a consultation in it.
Being self-pay is a deliberate structure, not a gap: no insurance-imposed restrictions on technique, surgical time, or anesthesia choice; a single all-inclusive written quote at your free consultation; and financing options if you want to spread the cost. If insurance coverage is essential to your plan, an in-network hospital program will serve you better — and we would rather tell you that here than after a consultation.
How a Plan Comes Together
Most gender-affirming journeys span more than one procedure, and sequencing matters — what heals first, what can be combined, what should wait. At a free consultation, Dr. Troell examines your anatomy, listens to which features matter most to you, and builds a staged written plan with a written fee for each stage. Nothing is sold as a package; some patients do one procedure and stop, others work through a full sequence over a year or more.
Because the whole plan lives with one surgeon in one office, each stage starts from an exam by the person who did the last one — not from a records transfer.

Transgender Cosmetic Surgery FAQs
Yes. We are an LGBTQ+ friendly and transgender-safe practice — the same commitment we state on our homepage — and we welcome all patients with respectful, confidential care. Your name and pronouns are used as you give them, at every visit.
Facial feminization (brow lift, feminizing rhinoplasty, custom chin and jaw implants, facial fat grafting, facelift, and tracheal shave), MTF breast augmentation with implants or your own fat, FTM top surgery (masculinizing chest surgery), and body feminization — hip and buttock contouring with implants or fat grafting plus VASER waist sculpting.
No. Dr. Troell does not perform genital surgery, voice surgery, or hormone management. We focus on the facial, chest, and body procedures listed on this page, and we are honest about that scope — if what you need is outside it, we will say so at consultation rather than stretch.
Our patients are adults — 18 and older for surgical procedures, and 22 and older for silicone gel breast implants per FDA guidance. Readiness, health history, and goals are reviewed one-on-one with Dr. Troell at a free consultation.
That worry is fair — gender-affirming care is one of the most travel-heavy corners of surgery, and aftercare from 1,500 miles away is a real problem when a journey often spans more than one procedure over many months. Dr. Troell has held an active Nevada medical license since 2001 and has practiced in Las Vegas for more than 20 years. He is the only surgeon at Troell Cosmetic Surgery, and consultation, surgery, and every follow-up visit happen in the same office on South Fort Apache Road. Whether a question comes up at week one or month nine, the surgeon who operated on you is the one who sees you — here, not in another state.
No — Troell Cosmetic Surgery is a self-pay specialty surgical practice and does not bill insurance for any procedure. We know many patients pursue insurance coverage for gender-affirming care, so we say this plainly up front. What self-pay means here: a transparent, all-inclusive written quote at consultation, no insurance-imposed restrictions on technique or surgical time, and financing options if you want them.
