A lip lift is a surgical procedure that shortens the philtrum — the vertical distance between the base of the nose and the top edge of the upper lip. As the philtrum shortens, the upper lip rotates slightly outward, revealing more of the pink vermillion edge and creating a fuller, more youthful lip appearance.
Unlike lip fillers, which add temporary volume to the existing lip body, a lip lift changes lip proportion. The result is permanent and addresses an aspect of lip aesthetics that filler cannot — the relationship between the nose, the upper lip, and the visible pink lip edge.

Three Lip-Lift Techniques

Bullhorn (Subnasal)
The most common technique. A discreet incision is placed in the natural shadow at the base of the nose. A small strip of skin is removed to shorten the philtrum and rotate the lip outward. Scar healing is excellent — most patients have an essentially invisible result within a few months.
Italian (Endonasal)
Incisions are placed inside the nostril sills (inner base of each nostril) for maximum scar concealment. Not all patients are anatomically suitable, but for those who are, the Italian technique produces no externally visible scar.
Direct (Vermillion)
An incision placed along the vermillion border itself. Less common today — typically reserved for specific anatomical or revisionary cases where the bullhorn technique would not produce the right result.
Are You a Lip Lift Candidate?
Strong candidates for a lip lift typically have:
- A long philtrum. The vertical distance between the nose and the top of the upper lip exceeds what's aesthetically ideal for the patient's face shape.
- A thin or "missing" upper lip vermillion. The pink edge of the upper lip is not visible at rest, even when smiling.

What to Expect After a Lip Lift
- Days 1-3. Mild swelling and tightness around the upper lip and nose base. Most patients return to desk work within 2-3 days.
- Week 1. Sutures are removed at day 5-7. The upper lip will feel tight; smiling and certain mouth movements feel restricted but normal.
- Weeks 2-4. Most visible swelling resolves. The scar appears pink and slightly raised; this is expected and improves steadily.
- Months 2-6. Final shape settles. Scar continues to mature — becoming progressively less visible over the first year.
- One year. Scar is typically essentially invisible; final lip appearance is fully settled.
